Subcontractor Requirements
- Subcontractors must sign a contract based on their estimate and adhere to the general conditions of the agreement. View Sample here
- New subcontractors must complete a Subcontractor Information Form and W-9 prior to starting work on their first job for CMHFH. View Sample SIF.
- Comply with CMHFH's timeline for completing work on the project.
- Must be registered with the MN Dept. of Commerce as required, or the respective licensing agency.
